Who must file Form 1095-C with the IRS?

Under the Affordable Care Act (ACA), Form 1095-C must be filed and distributed by Applicable Large Employers (with 50 or more full-time equivalent employees) to all full-time equivalent employees who were enrolled in health insurance coverage for at least one month of the calendar year.
